Dear Parents/Carers

We have been asked by Essex Partnership Trust Community and School Aged Immunisation Service to host the vaccination sessions for 12-15 year olds on Tuesday 5 and Wednesday 6 October.

No student will receive the vaccination without parental consent. If you wish for your child to receive their vaccination you must complete the electronic consent form in the attached letter by Monday 4 October.

Apologies for the short deadline, we have only just received the letter.

Any queries or questions about the process, please direct to the Essex Partnership Trust Community and School Aged Immunisation Service as this is not a school organised event.
